Consent to Tattoo
I acknowledge by signing this agreement that I have been given the full opportunity to ask any and all questions which I might have about the obtaining of a tattoo and that all of my questions have been answered to my full satisfaction. I specifically acknowledge I have been advised of the facts and matters set forth below and I agree as follows:
I agree to receive a random tattoo design from the flash displayed on the "Get What You Get" Banner.
I declare that I am the legal age of 18, I have provided valid proof of age, and I am competent to sign this agreement.
I am not pregnant or nursing.
I am not under the influence of alcohol or drugs. I understand that if I show up to my tattoo session under the influence, that I will NOT be tattooed that day, no exceptions.
I acknowledge that if I am wearing numbing cream that I have gotten the consent from my tattoo artist beforehand. I also acknowledge that numbing cream may increase the time needed to complete the tattoo, which will be at my own expense.
I acknowledge that my decision to obtain my tattoo is of my own free will and choice. I consent to the location of the tattoo, the performance of the tattoo procedure, and to any actions or conduct of the representatives and employees of RVA Tattoo Arts LLC necessary to perform the tattoo procedure.
I acknowledge and agree to follow any and all instructions provided to me regarding the maintenance of a sanitary environment while I am being tattooed.
I agree to IMMEDIATELY notify my tattoo artist if I feel lightheaded, dizzy, and or faint before, during, or after the procedure. I understand that failure to do so releases RVA Tattoo Arts LLC and my artist of all responsibility.
I acknowledge that it is not reasonably possible for the representatives and employees of Loose Screw Tattoo to determine whether I might have an allergic reaction to the dyes, pigments or process used in my tattoo. I agree to accept the risk that such a reaction is possible.
I do not have epilepsy, hemophilia, a heart condition, diabetes, take blood thinning medication, nor am I the recipient of an organ or bone marrow transplant. I acknowledge that if I have any of the above that I have consulted with a physician and I have been given the approval to proceed with the tattoo application. I understand the risks and if I encounter problems with my tattoo related to my illnesses, I release RVA Tattoo Arts LLC of responsibilities.
I do not have medical or skin conditions such as, but not limited to: ance, scarring (keloids), eczema, psoriasis, freckles, moles, or sunburn in the area to be tattooed that may interfere with said tattoo. If I have any type of infection or rash on my body I will advise my tattooer.
I understand that if I have any skin regimens such as: laser hair removal, plastic surgery, or other skin altering procedures, it may result in adverse changes to my tattoo.
To my knowledge I do not have physical, mental, or medical impairment / disability which may affect my well being as a direct or indirect result of my decision to have a tattoo.
I acknowledge that variations in color and design may exist between any tattoo as selected by me and as ultimately applied to my body. I understand that if my skin color is dark, certain colors of ink will not appear as bright as they do on light skin.
I understand that over time the color and clarity of my tattoo will fade due to unprotected exposure to the sun and the naturally occurring dispersion of pigment under the skin.
I acknowledge that I have been fully informed of the inherent risks associated with getting a tattoo. I fully understand that these risks, known and unknown, can lead to injury, including but not limited to: infection, scarring, and difficulties detecting melanoma. I understand it's possible to have an allergic reaction to tattoo pigment, latex, gloves, and / or soaps. Having been informed of the potential risks associated with getting a tattoo, I still wish to proceed with the application and I freely accept and expressly assume any and all risks that may arise from tattooing.
I acknowledge that the artist or employee/volunteer of RVA Tattoo Arts LLC has given me the fullest opportunity to ask any and all questions about the application of my tattoo and all of my questions have been answered to my total satisfaction.
I acknowledge that infection is always possible as a result of obtaining a tattoo. Particularly in the event that I do not take proper care of my tattoo. I have received aftercare instructions and I agree to follow them while my tattoo is healing. I agree that any touchup work needed, due to my own negligence, will be done at my own expense. This includes sun and UV damage
I acknowledge that I have been given adequate opportunity to read and understand this document and I understand that I am signing a legal contract waiving certain rights to recover against RVA Tattoo Arts LLC.
I acknowledge that I am free of communicable disease
I understand that there are certain factors that may change the time it takes to finish a tattoo session, included but not limited to: skin texture, tolerance levels, need for breaks, cooperation, whether or not numbing cream had been applied, changes in size, detail, and body placement.
I fully understand THE TATTOO ARTIST DOES NOT ACT AS A MEDICAL PROFESSIONAL. Any suggestions made to me are NOT to be construed as or substituted for advice from a medical professional.
I acknowledge that the procedure of tattooing can be a very invasive and emotional experience for me. I understand that it is imperative that RVA Tattoo Arts LLC is fully informed of any health conditions or matters that may surface during this process. I understand that if I feel I may be at risk during the act of the tattoo, I should consult with a physician before my appointment.
I understand that a tattoo is a permanent change to my appearance and can only be removed by laser or surgical means, which can be disfiguring and/or costly and which in all likelihood will not result in the restoration of my skin to its exact appearance before being tattooed.
I acknowledge that I have been informed by my tattoo artist that if there is an extensive amount of skin that is covered during the application of a larger tattoo it may be necessary to apply multiple layers of ink into various portions of the tattoo in order to achieve the desired saturation.
I release all rights to any photographs or videography taken of me and the tattoo and given consent in advance to their reproduction in print or electronic form.
If I have any allergies to latex or other materials/products I will let the tattoo artists know before they start the procedure.
